Sometimes despite our best efforts to treat endodontic problems conservatively there remains a non-healing situation. There may be reasons canals cannot be negotiated by going thru the tooth.
These may include:
1. calcification in canals
2. broken instruments
3. severely curved canals
4. ledged canals
 
If a tooth has been treated endodontically and for some reason remains non-healing then a surgical approach to the root end may be necessary to save the tooth. So if we cannot remedy a problem by going up inside the roots we can often solve the problem by approaching the root from the other direction. In a surgical endodontic procedure we make a small incision in the gum tissue and reflect it to expose bone and tooth root structure. Usually the root tip is removed and a root end cavity is prepared and filled with a biocompatible material. This puts a seal in the end of the root like putting a cork in a bottle so it won’t leak.
